How do I request a form from a patient or family?

Open the patient’s Family conversation and use Send form in the composer when available. Choose a supported template and review the request before sending.

How do I complete a requested form?

Open the requested form from the Family conversation in the portal. Complete all required fields and submit. Return to the conversation after the submission succeeds.

Why won't the form submit?

Check the validation message and complete the required fields in the requested format. Confirm you still have access to the patient room. If it says the form was already submitted, do not keep resubmitting.

Someone else already submitted the form. Can I submit it too?

The app accepts one submission per form request, not one per family member. Ask the care team for a new request if another response or a correction is needed.

Can I fill in a patient/family form on their behalf from this screen?

Only patient and family accounts can submit these forms. Staff can send a request and review the response.
